Beginning-of-the-Year Forms

Beginning-of-the-Year Forms are available electronically through the Sapphire Parent Portal for your convenience.

After logging in to the Parent Portal, select your student and click “Student Data Forms” on the left-hand side of the screen. Please complete all required beginning-of-the-year forms for each student in your household.

As part of the beginning-of-the-year process, families should review and complete the following important forms:

* District Device Insurance Form – Please review and complete this form for each student who will be issued a district device. This form provides information regarding optional insurance coverage for district-issued devices. This form is due by Wednesday, September 9, 2026. No payments will be accepted after that date.

* Family Income Survey – Please complete the Family Income Survey for your household. Information collected through this survey helps the District determine eligibility for programs, services, and funding that directly benefit our students and schools.

* Yearly Medical Form – Please complete the Yearly Medical Form for each student to ensure the District has current health and medical information for the 2026–2027 school year.

* Student Contact Information – Please carefully review and update your student’s contact information, including parent/guardian information, phone numbers, email addresses, and emergency contacts. Having accurate information is essential so the District can reach you when needed.

If you have more than one student enrolled in the District, please remember to select each student individually and complete all required forms for each child.

If you are unable to complete the forms electronically or do not have access to a Sapphire Parent Portal account, please contact your child’s school office for assistance or to request a physical copy.

Attendance Excuses

Parents/guardians may submit student attendance excuses electronically through the Sapphire Parent Portal throughout the 2026–2027 school year.

To submit an excuse, use the On-Demand Attendance Form available in Sapphire. This form may be used throughout the school year to submit documentation for your student’s absence.
