The Millville School Physician will be conducting a comprehensive medical exam for students in Kindergarten, sixth grade, and eleventh grade on Thursday, September 20^th in the Elementary School. A privately conducted physical examination completed within one year of students entry into the grade where an exam is required is acceptable and encouraged. Parents/Guardians, if you have not completed the paper form sent home with student, please login to SAPPHIRE and complete school physical permission form, or request a paper copy from the school nurse. If no response is received, and student is not within compliance, a medical exam will be conducted by the school physician.  Please contact the school nurse with any questions. Thank you.
